Certificate of Business Liability Insurance

Jasper County Merchant License

Jasper County Health Department Inspection (Annual or Temporary)

City of Carthage Business License ($50 annual I $25 after July 1)

Occupancy Inspection - Carthage Public Works ($25)

All documents and payment must be submitted to Experience Carthage, MO prior to the first event attended.
